Most MMO forms are available at www.mybrokerlink.com.
You can set your own password. Once inside, go to the "Producer's Guide." You
will find "Forms-Quick References" to be a very helpful tool for
forms in PDF format. Benefit Highlight sheets are still available at www.mmoh.com under "Employer
Services."
Anthem
Forms
Go to www.Anthem.com.
Select "Brokers & Producers",
then choose Ohio from the state
options and click on enter site.
On the far left side of the page under "Answers @ Anthem", select "Individual
Producer Web Site", then select "New User Registration" and follow
the
prompts.
You must register first to be able to use this secure site.
Nationwide
Forms
Nationwide forms are available online at www.nationwidehealthplans.com.
If you prefer Carnegie
Brokerage mail forms to your office, enter the quantity
you would like sent to you for each form and click
on the "Submit" button
at the bottom of the page. Your request will automatically
be sent to Carnegie Brokerage Agency.
